Klomifen sitrat ile indüklenen sikluslarda eksojen östrojen kullanımının folliküler ve luteal faz fonksiyonları üzerine etkisi

Abstract

Klomifen sitrat (CC), ovulasyon indüksiyonu amacıyla çok yaygın olarak kullanılmasına karşın, elde edilen ovulasyon oranı %85 iken gebelik oranı %40'dır. Bu fark, klomifen s it ratın hedef organlarda, özellikle serviks ve endometrium üzerinde belirgin olan antiöstrojenik etkisiyle açıklanır. Bu çalışma 34'ü primer infertilite, 14'ü sekonder infertilite tanısı almış olan ve bunların 38'inde anovulasyon, 10'unda oligoovulayon saptanan 48 hasta ile yapıldı. 24 hastaya sadece klomifen sitrat, diğer 24 hastaya ise CC ve konjuge östrojen verildi. Çalışma grubunda 14. gün serum testosteron düzeyleri düşük, serum estradiol düzeyleri yüksek saptandı (p<0.05). Çalışma ve kontrol grupları arasında, 21. gün progesteron düzeyleri ve luteal yetmezlik düşünülen olgular açısından görülen fark istatistiksel açıdan anlamlı değildi (p>0.05). 14. gün serum T düzeyleri ile diğer parametreler karşılaştırıldığında iki grup arasında istatistiksel açıdan anlamlı farklar gözlendi (p<0.05). Bu sonuçlara göre klomifen sitrat ile indüklenmiş sikluslarda, 0.625 mg konjuge östrojenin rutin olarak kullanılmasını öneriyoruz.

Clomiphene citrate is commonly used for ovulation induction. While the ovulation rate is 85% in stimulated cycles, the pregnancy rate is 40%. The lower pregnancy rates is the result of anti-estrogenic effect of the clomiphene citrate on target organs such as endometrium, and cervix. This study included 48 patients with anovulation. Twenty-four patients received only clomiphene citrate (CC), the remaining 24 patients received CC plus conjugated estrogen. The levels of serum testosteron (T) were low on 14th day, but the levels of serum estradiol (E2) were high in the studied group (p<0.05). Based on the levels of serum progesterone on 21th day and the incidence of luteal phase defect, there was no significant difference between both groups (p>0.05). When the levels of serum T on 14th day were compared with the other parameters, there was a significant difference between both groups (p<0.05). Based on the above results, we routinely recommend the use of 0.625 mg conjugated estrogen in CC stimulated cycles.
