Hekimlerin akılcı laboratuvar kullanımı hakkında bilgi düzeylerinin değerlendirilmesi

Abstract

Amaç: Gereksiz ve uygunsuz istenen tetkikler laboratuvarlarda maliyet ve iş yükü artışına neden olmaktadır. Laboratuvarların akılcı kullanılması ve gereksiz test istemlerinin azaltılması için çeşitli yöntemler geliştirilmiştir. Bu çalışmada hekimlerin akılcı laboratuvar kullanımı hakkındaki farkındalıklarını ve klinik pratikte sık istenen testlere ait test istem periyodu hakkındaki bilgi düzeylerini ölçmek amaçlandı. Materyal ve Metot: Çalışma kesitsel tanımlayıcı bir çalışma olarak planlandı. Bu kapsamda çalışmaya Turgut Özal Tıp Merkezi'nde çalışmakta olan hekimlere yönelik 'Akılcı Laboratuvar Kullanımı' üzerine bir anket uygulandı. Hekimlerin yaş, cinsiyet, branş ve meslekteki çalışma süreleri sorgulandı. Çoktan seçmeli sorularla hekimlerin sık istenen bazı testlerin istem periyotlarına yönelik bilgi düzeyleri sorgulandı. Pearson chi square ve Mann Whitney U test ile veriler analiz edildi. Bulgular: Dahili ve cerrahi braşlarda toplam 400 hekime anket uygulandı. Hekimlerin yaş ortalaması 33,01±5,97 (min=22, maks=59) yıl idi. Hekimlerin %3,3'ü, kan lipitleri, %80,8'i HbA1c, %64,5'i Üre/kreatinin, %40'ı D vitamini ve B12 vitamini istem periyodu hakkındaki soruyu doğru yanıtladı. Hekimlerin %85,3'ü akılcı laboratuvar test istem prosedüründen haberdar değildi ve %94'ü akılcı laboratuvar kullanımı ile ilgili herhangi bir eğitim almamıştı. Sonuç: Hekimlerin akılcı laboratuvar kullanımı hakkındaki bilgi düzeyleri yetersizdir ve tsetlerin istem periyodları hakkında eğitim almaya ihtiyaçları vardır. Evaluatıon Of Knowledge Levels Of Physıcısts About Ratıonal Laboratory Use Objective: The unnecessary and inappropriate tests required increase the cost and workload in the laboratories. Various methods have been developed for rational use of laboratories and reduction of unnecessary test requests. In this study, it was aimed to measure the awareness of physicians about rational laboratory use and the level of knowledge about the test request period for frequently requested tests in clinical practice. Marial and Method: The study was planned as a cross-sectional descriptive study. In this context, a questionnaire on 'Rational laboratory use' was applied to the physicians working at Turgut Özal Medical Center. The age, gender, specialties and professional experiences of the physicians were questioned. With multiple choice questions, the knowledge levels of the physicians regarding the demand periods of some frequently requested tests were questioned. The data were analyzed by Pearson chi square and Mann Whitney U test. Results: A total of 400 physicians were surveyed in internal and surgical braches. The average age of the physicians was 33.01 ± 5.97 (min = 22, max = 59) years. 3.3% of physicians answered the question about blood lipids, 80.8% HbA1c, 64.5% Urea / creatinine, about 40% vitamin D and vitamin B12 request period. 85.3% of the physicians were not aware of the rational laboratory test request procedure and 94% had not received any training on rational laboratory use. Conclusion: The level of knowledge of physicians about rational laboratory use is insufficient and they need to be trained about the test periods of tests.
