Psöriatik artrit hastalarında işgücü kaybı ve ilişkili durumların değerlendirilmesi

Abstract

Amaç: Psöriatik artrit (PsA) sakroileit, spondilit, entezit, daktilit ve artrit şeklinde kas iskelet tutulumu yapabilen psörizis ile ilişkili kronik inflamatuar bir hastalıktır. Psöriatik artrit hastalarını etkileyen en önemli sorunlardan birisi de işgücü kaybıdır. Özellikle kas iskelet tutulumu, palpo-plantor bölgelerin tutulumu, yorgunluk, kaşıntı semptomları ve uykusuzluk nedeniyle bu hasta grubunda işgücü kaybı beklenmektedir. Biz de bu çalışmada psöritik artrit hastalarında işgücü kaybını ve buna neden olan durumları değerlendirdik. Meteryal ve Metot: 50 PsA hastası, 50 kontrol grubu alındı. Tüm hastalar uykusuzluk şiddet indeksi, VAS ağrı skorlama, FACİT, DAPSA, PASİ, NAPSİ, LEİ, HAD ve iş gücü kaybı açısından değerlendirildi. Rutin tetkiklerden olan eritrosit sedimentasyon hızı Westergren yöntemi (mm/saat), CRP ise nefelometrik (mg/dl) olarak ölçüldü. Bulgular: Hasta ve kontrol grubunda çalışma yüzdesi ve genel işsizlik yüzdesinde açısından anlamlı fark saptanmadı (p>0,05). Verimlilik kaybı yüzdesi (p=0,024) ve sağlığa bağlı aktivite yüzdesinde (p=0,00) hasta ve kontrol grubunda anlamlı farklılık saptandı. Günlük faaliyetlerde azalma yüzdesi ile hassas eklem sayısı (p=0,002), Doktor VAS puanı (p=0,001), hasta VAS puanı (p=0,000), DAPSA (p=0,005), LEİ (p=0,002) ve sedimantasyon (p=0,044) arasında anlamlı farklılık saptandı. Sonuç: Çalışmamızda daha çok günlük aktivitelerde etkilenmeler gözlemledik. İş zamanındaki kayıp sınırlıydı. Ancak uykusuzluk, yorgunluk ve entezit olan hastalarda günlük işlerinde etkilenmenin haricinde iş verimliliğinde de azalma gözlemledik. Anahtar Kelimeler: Psöriatik artrit, İş gücü kaybı, Verimlilik

Aim: Psoriatic arthritis (PsA) is a chronic inflammatory disease associated with psoriasis capable of musculoskeletal involvement in the form of sacroiliitis, spondylitis, enthesitis, dactylitis and arthritis. Managing patients with psoriatic arthritis is one of the most important problems. Especially due to musculoskeletal involvement, palpo-plantor region involvement, fatigue, itching symptoms and insomnia, loss of labor is expected for this patient. We evaluated the loss of labor and the situations that caused it in arthritis patients Material and Method: 50 PsA patients, 50 control groups were taken. All patients were evaluated in terms of insomnia severity index, VAS pain scoring, FACİT, DAPSA, PASI, NAPSI, LEI, HAD and loss of labor. The erythrocyte sedimentation rate, which is one of the routine examinations, was measured as Westergren method (mm/hour), and CRP was measured as nephelometric (mg/dl). Results: There was no significant difference in terms of the percentage of work and overall unemployment in the patient and control groups (p>0,05). A significant difference was found in the patient and control groups in the percentage of efficiency loss (p=0,024) and the percentage of health-related activity (p=0,00). Percent reduction in daily activities and the number of sensitive joints (p=0,002), Doctor VAS score (p=0,001), patient VAS score (p=0,000), DAPSA (p=0,005), LEI (p=0,002) and there was a significant difference between sedimentation (p=0,044). Conclusion: In our study, we observed mostly effects on daily activities. The loss in work time was limited.
