Orta persistan astımlı hastaların tedavisinde nedokromil sodyumun yeri

Abstract

Öz:Nedokromil sodyum (NDS); hafif persistan astımda semptom kontrolünü sağlamak amacıyla kullanılan güvenilir ve etkili bir anti-inflamatuar ajandır. Amaç: Bu çalışma, orta persistan astımlı hastaların tedavisinde NDS'nin rolünü araştırmak amacı ile randomize, prospektif ve plasebo kontrollü olarak yapılmıştır, Materyal ve Metot: Polikliniğimize ardışık olarak başvuran orta persistan astımlı 30 hasta, randomize üç gruba ayrılmıştır. Birinci gruba (n=11) inhale 1800 mcg/gün budesonid ve uzun etkili inhale bir b2 agonistin (Salmeterol) yanı sıra lüzumu halinde kullanacakları kısa etkili inhale b2 agonistten (Terbutalin) oluşan standart tedavi verilirken, ikinci gruba (n=10), birinci grubun kullandıklarına ek olarak plasebo NDS, üçüncü gruba (n=9) birinci grubun tedavisine ilaveten NDS verildi. Hastaların tümü;birinci hafta, birinci ay ve üçüncü ayda kontrollere çağrılarak başlangıçta ve kontrollerinde spirometrik ölçümleri yapıldı. Tüm hastalardan;semptomlarını ve kısa etkili p2 agonist ihtiyaçlarını yazmaları için günlük bir form doldurmaları istenerek, her kontrolde semptom skorla mal arı yapıldı, Bulgular: Gruplar arasında yaş ortalamaları ve ortalama hastalık süreleri açısından fark yoktu (p > 0.05). Birinci, ikinci ve üçüncü gruplarda başlangıç semptom skorları sırasıyla 13.00, 12.80 ve 12.88 iken (p>0.05), üçüncü ayın sonunda semptom skorları 5.71, 5.87 ve 7.00 bulundu (p>0.05). Gruplar arasında hem başlangıç, hem de tedavi sonuçları göz önüne alındığında iyileşme oranlan açısından FVC, FEV1, FEV/FVC ve FEF25-75 parametrelerinde istatistiki olarak anlamlı farklar saptanamadı (p>0,05), Sonuç: Başlangıçtaki ve çalışmanın sonundaki semptom skorları-solunum fonksiyon testleri kıyaslandığında, NDS'nin orta persistan astımlı hastaların tedavisine olumlu bir katkısı bulunmamıştır, Başlık (İngilizce):Utility of nedocromyl sodium in the treatment of moderate persistent asthmatic patients Öz (İngilizce):Nedocromyl sodium (NDS), used to control symptoms in the treatment of mild persistent asthma, is a safe and effective anti-inflammatory agent, Objective: This study was planned as prospective, randomised and placebo controlled to determine the efficacy of NDS in patients with moderate persistent asthma, Material and Method: Thirty patients with moderate persistent asthma, who consecutively applied to our clinics, were randomised into three groups. While prescribing the first group (n=11) a standard regimen including budesonide 1800 meg/day, long acting b2 mimetic (salmeterol), and a short acting b2 mimetic (terbutalin) when necessary, the second group (n=10) was given the same treatment of the first group and additional placebo NDS and the third group (n=9) was given the same treatment of the first group and NDS itself. All patients were called for the controls at the first week, the first month and the third month, and their spiro metric examination was done during these controls and at the beginning. All patients were asked to fill a daily registry card questioning their symptoms and need for short acting p2 mimetic, then their symptom scoring was done at every control. Results: Regarding the mean age and mean duration of the disease, there was not any statistically significant difference between the groups (p>0.05) While their mean symptom scores were 13.00, 12.80, and 12.88 respectively at the beginning, it became 5.71, 5.87, 7.00 respectively at the and of the third month (p>0.05). Considering their initial and final values of FVC, FEV1, FEV/FVC and FEF25-75, there was no statistical significant difference between the groups (p>0.05). Conclusions; comparing initial and final symptom scores and spirometric parameters, no additional positive effect of NDS in the treatment of the patients with moderate persistent asthma could not be demonstrated.
