Üst ekstremite ve bel ağrısı tanısı konulmuş sedanter kadınların katılımı ile gerçekleştirilen bu çalışma, kalistenik egzersizlerin ağrı eşiği, ağrı şiddeti, kas kuvveti, vücut komposizyonu, esneklik üzerine etkilerini incelemek amacı ile yapılmıştır. Çalışma grubumuzu Malatya ilindeki özel bir hastanenin fizik tedavi polikliniğine başvuran, yaşları 40-60 arasında değişen, üst ekstremite ve bel ağrı tanısı konulan, sedanter 80 kadın oluşturmuştur. Denekler rastgele iki gruba ayrılmıştır. Kontrol grubunda 40 deneğe konservatif tedavi (ultrason (US), hotpack (HP), tens), eğitim grubunda 40 deneğe kalistenik egzersiz ve konservatif tedavi uygulanmıştır. Her iki gruba ön test- son test modeli uygulanmıştır. Tedavi öncesi ve uygulanan tedaviler sonrası deneklere ağrı eşiği, ağrı şiddeti, kas kuvveti, vücut komposizyonu, esneklik testi yapılmıştır. Veriler SPSS paket programı ile değerlendirilmiş ve anlamlılık düzeyi p<0.05 olarak alınmıştır. Yapılan çalışma sonunda, konservatif tedavi ile kalistenik egzersizlerin bir arada uygulandığı terapilerde kas kuvveti, ağrı eşik değeri, esneklik değerlerinde artış görüldüğü (p<0.05), ağrı şiddeti değerlerinde ve bel/kalça oranı değerlerinde azalma görüldüğü (p<0.05) tespit edilmiştir. Konservatif tedaviye ek olarak uygulanan kalistenik egzersizlerle birlikte bütün kas kuvvet değerlerinde tedavi sonrasında tedavi öncesine göre artış gözlemlenmiştir. Kas kuvvet değerleri ile ağrı eşik değerleri arasında iki grup arasında anlamlı farklar bulunmuştur (p<0.05). Yapılan çalışmada, servikal 3, servikal 5, lumbal 3 ve lumbal 5 ağrı eşiği değerleri ile sırt ekstansörleri ve abdominal kaslar arasında anlamlı ilişkiler bulunmuştur. Sonuç olarak; konservatif tedavi ile birlikte uygulanan kalistenik egzersizlerin kas kuvvetini artırarak, ilgili kasın ağrı eşiği değeri üzerinde etkili olduğu düşüncesi gelişmiştir. Üst ekstremite ve bel ağrı şikâyeti olan sedanter kadınlarda kas kuvvetinin ağrı eşiği değerlerini etkilediği, bel ve boyun bölgelerinden alınan ağrı eşiği değeri ile karın ve sırt kas kuvvetleri arasında ilişkilerin olduğu gösterilmiştir. Bu nedenle kas kuvvet artışı sağlayan uygulamaların ağrı ile baş etmede etkili olacağı kanaatine varılmıştır.
This study that carried out with the participation of sedentary women who are diagnosed with upper extremity and low back pain, was conducted to analyze the effects of calisthenic exercises on pain threshold, pain severity, muscle strength, body composition, and flexibility. Our study group was included 80 sedentary women, age range from 40-60, who are diagnosed with upper extremity and low back pain are admitted to a private hospital for outpatient physical therapy clinic in Malatya. Patients were randomly assigned to two groups. 40 patients underwent conservative therapy (US, HP, tens) in control group and 40 patients received conservative therapy and calisthenic exercises in training group. Both groups were administered pre test and post test model. The subject's pain threshold, pain severity, muscle strength, body composition and flexibility tests were assessed before and after the treatment. The data were evaluated using SPSS package program and the level of significance was taken as p<0.05. Based on the results of this study, therapies that conservative treatment and calisthenic exercise were applied together muscle strength, pain threshold, and flexibility values were significantly increased (p <0.05), pain intensity values and waist/hip range values were significantly decreased (p <0.05). All muscle strength variables were observed significantly improvement compared to baseline values after treatment with calisthenic exercise added to conservative treatment. There were significant differences found in muscle strength and pain threshold values between the two groups (p<0.05). In this study, there was significant relationship found between pain threshold values of the cervical third, cervical fifth, lumbar third, lumbar fifth and abdominal muscles and back extensors. In conclusion; the idea was developed that conservative treatment administered with calisthenic exercise increased muscle strength related to increase muscle pain threshold values. This study points out that muscle strength affects pain threshold values received from waist and neck region related with the abdominal and back muscle strength in women with upper extremity and low back pain. Therefore; we concluded that interventions increasing muscle strength would be effective in coping with pain.
