Karaciğer sirozlu hastalarda portal basıncın yükselmesi ve hiperdinamik dolaşım, asitin oluşumunda rol oynayan faktörlerdir. Her iki durumun ortaya çıkmasında nitrik oksitin (NO) etkisini gösteren çalışmalar mevcuttur. Nitrik oksit sentaz (NOS) geni polimorfik yapıda olup bu gendeki polimorfizmlerin nitrik oksit sentez ve salınımını değiştirdiği tesbit edilmiştir. NOS enziminin 3 izoformundan birisi olan endotelyal nitrik oksit sentaz (eNOS) genine ait tanımlanmış polimorfizmlerden bazıları sirotik portal hipertansiyonla ilişkili bulunmuştur. Biz bu çalışmada NO' nun salınımı ve etkinliğini değiştirdiği ispatlanmış olan eNOS geni G894T ve T786C polimorfizmleri ile sirotik hastalarda asit gelişimi arasında ilişki olup olmadığını araştırdık. Bu amaçla siroz tanısı almış ve asit gelişmiş olan 70 hasta, siroz tanısı almış ve asit gelişmemiş 69 hasta ile 60 sağlıklı bireyden oluşan kontrol grubu oluşturuldu. Standart metodlar kullanılarak periferik kandaki lökositlerden DNA analizi yapıldı. Polimorfizmlerin tespiti bir LightCycler-System ile floresan işaretli hibridizasyon probları kullanılarak erime egrisi analizi ile yapıldı. Sonuç olarak bu çalışmada eNOS geni G894T ve T786C polimorfizmleri ile sirozlu hastalarda asit gelişimi arasında bir ilişki bulunamamıştır. Anahtar kelimeler: siroz, asit, nitrik oksit, polimorfizm
Portal hypertension and hyperdynamic circulation are factors that play role in development of ascite in liver cirrhosis patients. There are some studies that show the effect of nitric ocide (NO) in both situation. Nitric oxide synthase (NOS) gene has a polymorphic structure. Also it was found that polymorphisms of this gene change the synthesis and secretion of nitric oxide. Some of the identified polymorphisms of Endothelial nitric oxide synthase, one of the three isoform of NOS, has been found to associated with cirrhotic portal hypertension. We investigated whether there is a relationship between eNOS gene G894T and T786C polimorphism, which were proven to alter the activity and release of NO, and development of ascite in liver cirrhosis. We formed three groups of patients; first group 70 patients who diagnosed cirrhosis with ascite, second group 69 patients who diagnosed cirrhosis withhout ascite and 60 healthy control group. DNA was isolated from peripheral blood leukocytes by standard methods. Detection of polymorphisms was done by rapid capillaryPCR with melting curve analysis using fluorescence-labeled hybridization probes in a LightCycler-System. As a result, it wasn't found any relationship between G894T and T786C polimorphism of eNOS gene and ascite development in liver cirrhosis. Key Words: cirrhosis, ascite, nitric oxide, polymorphism
