Amaç: Dekspantenol (Provitamin B5) pantotenik asidin biyolojik aktif bir alkol formudur ve dokunun içerisinde pantotenik asit'e dönüşür. Pantotenik asit hücre içindeki indirgenmiş glutatyon (GSH), Koenzim A (CoA) ve ATP sentezini arttırır. Bunlar oksidatif streste hücre savunmasının ana unsurlarını oluşturmaktadır. Dekspantenol antioksidan özelliğiyle antiinflamatuvar niteliğe sahip olmaktadır. Bu çalışmada, deneysel astım modeli oluşturulmuş farelerde dekspantenolün akciğerdeki histolojik ve biyokimyasal değişiklikler üzerine etkilerinin araştırılması amaçlanmıştır. Materyal ve Metod: 40 adet dişi BALB/c fare 10'arlı 4 gruba ayrılıp, her bir grup ayrı kafeslere konuldu. Grup 1: Kontrol, Grup 2: Kronik astım, Grup 3: Kronik astım + dekspantenol, Grup 4: Protokole maruz bırakılmadan sadece dekspantenol verilen grup olarak belirlendi. Kontrol grubundaki farelere 0. ve 14. günlerde % 0,9 NaCl 0,1 ml intraperitoneal olarak verdik. Sonrasında 21. günden başlanarak 8 hafta boyunca, haftada 3 gün, 30 dakika süreyle % 0,9 NaCl solüsyonunun nebülizasyonu tüm vücut inhalasyonu yoluyla uyguladık. Grup II'deki tüm farelere, çalışmanın 0. ve 14. günlerinde 10 μg/0,1 ml tavuk yumurtası ovalbumini intraperitoneal yolla uygulandı. Bu farelere, son immunizasyondan 7 gün sonra (21. günde) başlamak üzere, günde 30 dakika süre ile haftanın 3 günü, 8 hafta boyunca steril salin içerisinde % 2.5'lik ovalbumin solusyonundan oluşan aerosol inhale ettirildi. Normal salin içerisindeki % 2.5'lik ovalbumin solüsyonu, ultrasonik nebulizatörle inhale ettirildi. Grup III deki aynı deneysel astım modeli oluşturulmuş farelere, çalışmanın son haftasında intraperitoneal 500 mg/kg/gün 1x1 dozda dekspantenol verildi. Grup IV deki farelere ise protokole maruz kalmadan, çalışmanın son haftası günde bir kez intraperitoneal 500 mg/kg dekspantenol verildi. Protokol bitiminden sonra fareler sakrifiye edildi. Akciğer dokusu iki parçaya bölündü. Sağ akciğer biyokimyasal analizler için kullanıldı. Sol akciğer histolojik analizler için kullanıldı. Bulgular: Ovalbumin uygulanan fareler kontrol grubuyla karşılaştırıldığında, alveol duvar kalınlığının kontrol grubuna göre istatistiksel olarak anlamlı derecede arttığı (p<0.05) ve alveol yapılarının bozulduğu tespit edildi. Ayrıca bronşiyol ve damar duvarı çevresinde yoğun inflammatuvar hücre infiltrasyonu gözlendi. Astım grubunda izlenen diğer bir bulgu; orta ve küçük çaplı bronşiyol epitelindeki goblet hücre sayısının belirgin olarak artmasıydı (p<0.05). Astım + dekspantenol grubunda, alveoler duvar kalınlığı, bronşiyol-kan damarları çevresinde izlenen inflamatuvar hücre yoğunluğu ve bronşiyol epitelindeki goblet hücre sayısı astım grubu ile karşılaştırıldığında, azalmıştı (p<0.05). Dekspantenol verilmesi bulguların şiddetini astım grubuna göre azaltmasına rağmen, bazı alanlarda inflamasyon ve alveoler septal kalınlaşma gözlendi. Astım grubunda kontrol grubuna göre GSH, SOD, TAC ve CAT seviyeleri azalmıştı. Bununla birlikte, astım + dekspantenol ve dekspantenol grubunda ise GSH, SOD, TAC ve CAT seviyeleri artmıştı. Bu verilerdeki artış istatistiksel olarak anlamlıydı (p<0.001). Astım grubunda kontrol grubuna göre MDA, NO, TOS, OSİ, TNF-α, IL-6 ve IL-1β seviyeleri artmıştı. Astım + dekspantenol ve dekspantenol grubunda ise azalmıştı (p<0.001). Sonuç: Dekspantenolün astım tedavisinde faydalı olduğunu belirledik. Bu yüzden, dekspantenol gelecekte astım tedavisinde primer, alternatif veya tamamlayıcı bir tedavi olarak kullanılabilir. Objective: Dexpanthenol (Provitamine B5) is an alcoholic analog of pantothenic acid, and it is oxidized to pantothenic acid within the tissues. Pantothenic acid and its derivatives increase the level of reduced glutathione (GSH), Coenzyme A (CoA) and ATP synthesis within the cell. All these items play a major role in cellular defense. The antioxidant properties of dexpanthenol have antiinflammatory qualities. In this study, we aimed to investigate the effect of dexpanthenol on histological and biochemical changes in the lung of an experimental asthma mice model. Materials and Methods: 40 female BALB/c mice were divided into 4 groups each containing 10 rats and each group was placed in separate cages. Group 1: Control group, Group 2: Chronic asthma , Group 3: Chronic asthma + dexpanthenol, Group 4 was the only dexpanthenol group without exposing protocol. We injected intraperitoneally 0.1 ml of 0.9% NaCl on 0. and 14. days to mice of the control group. We have applied nebulization of 0.9% NaCl solution through whole body inhalation for 30 minutes, starting on the 21.day and 3 days a week, during the 8 weeks. In the group II 10 mg/0.1 ml chicken egg ovalbumin was intraperitoneally applied in all mice, 0. and 14. days of the study. Consisting of 2.5% ovalbumin aerosol in sterile saline solution was inhaled on these mice that was started on the 7 days after the last immunization (day 21) and 3 days a week for 30 minutes per day during the 8 weeks. 2.5% ovalbumin solution in normal saline was given by ultrasonic nebulizer for inhalation. In the group III, 500 mg/kg of dexpanthenol was intraperitoneally given at the last week of study on the same experimental asthma mice model. In the group IV, intraperitoneally 500 mg/kg of dexpanthenol per day was given to mice in the last week of study that were not exposured to protocol. After the end of protocol the mice were sacrificed. The lung tissues were divided into two parts. The right lung was used for biochemical analysis, the left lung was used for histological analysis. Results: In the mice which were applied ovalbumin, alveolar wall thickness increased significantly (p<0.05) and deterioration of the alveolar structure was pointed out when compare with contol group. Also intense inflammatory cell infiltration around the bronchioles and the vessel wall was observed. Another finding was observed in the asthma group; number of goblet cells in the middle and small bronchial epithelium were significantly increased (p<0.05). In the asthma + dexpanthenol group; alveolar wall thickness, inflammatory cell density around blood vessels of bronchiole, number of goblet cell at bronchial epithelium, was decreased when compared with asthma group (p<0.05). Despite dexpanthenol reduced severity of the findings compared with the asthma group, inflammation and alveolar septal thickening was observed at some places. In the asthma group level of GSH, SOD, TAC, and CAT were decreased according to the control group. On the other hand, level of GSH, SOD, TAC, and CAT were increased in the asthma + dexpanthenol and dexpanthenol group. The increase in these findings were statistically significant (P<0.001). Level of MDA, NO, TOS, OSİ, TNF-α, IL-6, and IL-1β were increased in the asthma group compared with the control group. These findings were decrased in the asthma + dexpanthenol and dexpanthenol group compared with control group (P<0.001). Conclution: We pointed out that dexpanthenol is useful for treatment of asthma. Therefore, dexpanthenol can be used in treatment of asthma as primary, alternatively or complamentary in the future.
