Multipl skleroz; SSS'nin demiyelinizasyon ile giden kronik inflamatuvar otoimmün hastalığıdır. Etyolojide çevresel ve genetik faktörlerin karşılıklı etkileşimleri olduğu düşünülmektedir. Genellikle 20-40 yaş arasında bulunan genç erişkinleri etkilemektedir. Özellikle reprodüktif dönemdeki kadınlarda erkeklere oranla 3 kat daha fazla görülmektedir. Fingolimod; koruyucu tedaviler içerisinde kullanıma ilk giren oral ajan olması sebebiyle dikkat çekicidir. Hücre zarında bulunan sfingozin molekülüne olan benzerliği sayesinde beyin, kardiyak, endotel hücreleri, böbrek ve karaciğer gibi birçok dokuda bulunan sfingozin 1 fosfat reseptörlerine bağlanarak etkisini gösterir. Fingolimod esas olarak lenfoid dokulardan T hücrelerinin periferik dolaşıma çıkışını engelleyerek etki etmektedir. İlaca bağlı sık görülen yan etkiler arasında olan lenfopeninin ilaç devamında takip edilmesi gerekmektedir.Özellikle 200 ml'nin altında seyreden lenfosit değerlerinde ilaca ara verilmesi yada kesilmesi önerilmektedir. Bu çalışmada fingolimod tedavisi sonrasında ilaç yan etkisi olan lenfopeninin monitörizasyonu için en uygun menstrual siklus döneminin belirlenmesi amaçlanmıştır. Çalışmaya Ekim 2016-Aralık 2016 tarihleri arasında İnönü Üniversitesi Nöroloji Kliniğinde takipli RRMS tanısı olan 41 kadın hasta ile 33 sağlıklı kadın birey dahil edilmiştir. Her iki gruptaki bireylerden menstrual siklusun üç farklı evresinde periferik venöz kan örnekleri alınmıştır. Bu dönemlere ait total lökosit, nötrofil ve lenfosit düzeyleri incelenmiştir. Bu çalışmasonucunda fingolimod tedavisi sonrası görülen lenfopeni takibinin menstrual siklusun herhangi bir döneminde yapılabileceği saptanmıştır.
Multiple Sclerosis is a chronic inflammatory autoimmune demyelinating disease of the Central Nervous System. It is thought that there are responsible interactions of environmental and genetic factors in etiology. It seems especially young adults, between 20 and 40 years of age. Generally women in the reproductive period are three times more than men. Fingolimod; is striking because it is the first oral agent to use in protective treatments. Through its similarity to the sphingosine molecule found in the cell membrane, it binds to sphingosine 1 phosphate receptors found in many tissues such as brain, cardiac, endothelial cells, kidney and liver. Lymphopenia, which is a frequent side effect of pharmacological treatment, should be followed up during the medication. It is suggested, that fingolimod is discountinuedbetween the lymphocyte counts, especially below 200 ml in blood values. In this study, it is aimed to determine the most appropriate menstrual cycle period for the monitoring of the lymphopenia, which is the advers effect of the drug, after fingolimod therapy. The study was performed in Inonu University Neurology Clinic between October 2016 and December 2016. 41 female patients with RRMS and 33 healthy female individualswere included in this study. In both groups, peripheral venous blood samples were taken from individuals at different stages of the menstrual cycle. Total leukocyte, neutrophil and lymphocyte levels were examined in these periods. As a result of this study, monitor of lymphopenia can be performed at any time during the menstrual cycle in the course of fingolimod therapy.
