Amaç: Safra yolu komplikasyonları (SYK), sık insidansı, uzun tedavi süreci, greft ve alıcı sağ kalımı üzerine olan etkileri nedeniyle karaciğer nakli sonrası en önemli komplikasyonlar arasında yer almaktadır. Yakın zamandaki raporlarda morbidite oranı % 50, mortalite oranının ise % 30 lara kadar çıktığı bildirilmiştir. Safra anastomozu komplikasyonlarının en önemli nedenlerinden birinin arteriel iskemi olduğu bilinmektedir., Merkezimizde son bir yıl içerisinde yapılan karaciğer nakillerinde, teleskopik safra yolu anastomoz tekniği ile, mukozal safra yolu anastomozu yapılan hastaların sonuçlarını sunmayı amaçladık Materyal - Metod: 2015 yılı içerisinde gerçekleştirilen toplam 203 karaciğer naklinden 56 sına teleskopik bilier anastomoz uygulandı. Bu hastalardan 50 si araştırma kapsamında değerlendirildi ve kontrol grubu da 52 hastadan oluşan standart anastomoz yöntemi hastalarından seçildi. Hastalar retrospektif olarak tarandı. Hastaların ilk üç aylık anastomoz komplikasyonları, Fisher Ki-kare ve Mann Whitney-U istatistiksel analiz yöntemleri ile karşılaştırıldı. P<0,05 değeri anlamlı kabul edildi. Bulgular: Her iki grup arasında demografik ve klinik veri olarak fark yoktu. Elli teleskopik rekonstrüksiyon hastasının 5 (% 10) inde anastomoz kaçağı saptandı. Standart anastomoz yönteminde bu sayı 13'tü (% 25) (p<0,05). Teleskopik anastomoz yönteminde 16 (% 32), standart anastomoz yönteminde ise 24 (% 46) hastada anastomoz darlığı saptandı. Toplam bilier komplikasyon ise, teleskopik yöntemde 20 (% 40), standart yöntemde 31 (%60) hastada ortaya çıktı (p<0,05). Sonuç: Teleskopik bilier anastomoz tekniği ile bilier anastomozun ana safra kanalının iç mukozasına yapılması, arteriel beslenme sorununa katkı sağlamaktadır. Anastomoz hattının daha iyi kan akımına sahip olması ile özellikle anastomoz kaçaklarının ve buna bağlı darlık oranlarının azalabileceğini düşünüyoruz.
Background: Biliary complications are one of the most important complications of liver transplantation because of the effect on recipient and graft survival, frequent incidency and long treatment period. It has been reported with % 50 morbidity and % 30 mortality rates in recent studies. It has been known that, one of the most important reason of biliary anastomosis complications is arteriel ischemy. We aimed to present the results of telescopic biliary anastomosis technique that performed to the mocosa of the main biliary duct. Patients and Methods: Fifty six telescopic biliary reconstruction has been applied to totally 203 of patients during 2015. Fifty of those and 52 of standart reconstruction patients were chosen to evaluate. Patients were scanned retrospectively. Statistical analyses made with chi-square and mann whitney-u tests fort he complications of first 3 months. P<0,05 was meanful. Results: There was no clinical and demografic difference between two groups. There were 5 (% 10) anastomotic leaks of 50 telescopic reconstruction. It was 13 (% 25) at the standart reconstruction group (p<0,05). Anastomosis stenosis was detected at 16 (%32) patients in telescopic reconstruction group, and 24 (% 46) detected in standart reconstruction group. Total biliary complications were 20 (% 40) at telescopic reconstruciton group, and 31 (% 60) at standart reconstruction group (p<0,05). Conclusion: Performing the biliary anastomosis to the mucosal side of the main biliary duct, makes arterial blood supply better. Thus, anastomotic leak rates decrease and in order to low rates of leaks, anastomotic stenosis rates can also decrease.
