ÖZET Amaç : Bu prospektif, gözlemsel çalışmanın amacı; spinal anestezi uygulanan, preeklamptik ve non-preeklamptik gebelerde, sezaryen operasyonu süresince ölçülen bölgesel serebral oksijen satürasyonu (rSO2) değerlerini karşılaştırmak ve preeklamptik gebelerde rSO2 değerlerinin spinal anesteziye bağlı hemodinamik değişikliklerden nasıl etkilendiğini tespit etmektir. Materyal-Metot : Çalışmaya 18-45 yaş arası, ASA II-III, gestasyonel haftası 32 ve üzerinde olan ve spinal anestezi altında sezaryen kararı verilen, preeklamptik (Grup P, n=24) ve non-preeklamptik (Grup N, n=25) olmak üzere toplam 49 gebe alındı. Rutin monitörizasyona ek olarak iki sensörlü INVOS™ ile rSO2 monitörizayonu yapıldı. Spinal anestezi sonrası, operasyon süresince sağ ve sol frontal serebral sensörlerden elde edilen değerler (rSO2sağ ve rSO2sol) kaydedildi. Ayrıca, hemodinamik parametreler, periferik oksijen satürasyonu, verilen efedrin miktarı, toplam kanama miktarı, karşılaşılan komplikasyonlar, fetal değerlendirme için cilt kesisi-bebek çıkış ve uterus kesisi-bebek çıkış süreleri, yenidoğanın 1. ve 5.dk Apgar skorları, umbikal kord kan gazı değerleri kaydedildi. Bulgular : Bazal rSO2sağ ve rSO2sol düzeyleri Grup P'de Grup N'den daha yüksek olmakla birlikte gruplar arasında istatistiksel anlamlı fark yoktu. Spinal enjeksiyon sonrası yapılan rSO2sağ ölçümlerinde gruplar arasında istatistiksel anlamlı fark bulunamazken; rSO2sol ölçümleri açısından spinal enjeksiyon sonrası 3. ve 5.dakikalarda gruplar arasında istatistiksel anlamlı farklılık tespit edildi. Grup P'de rSO2sol düzeylerinin daha yüksek olduğu gözlendi. Grup N'de yapılan grup içi karşılaştırmalarda; rSO2sağ düzeylerinde spinal enjeksiyon sonrası 1., 3., 5. ve 10. dakikalarda, uterus insizyonu sonrası ve plasenta çıkışı sonrası yapılan ölçümlerde bazal değere göre istatistiksel anlamlı düşme tespit edildi. rSO2sol düzeylerinde spinal enjeksiyon sonrası 1., 3., 5. ve 10. dakikalarda ve uterus insizyonu sonrası yapılan ölçümlerde bazal değere göre istatistiksel anlamlı düşme tespit edildi. Grup P'de yapılan grup içi karşılaştırmalarda; rSO2sağ ve rSO2sol düzeylerinde spinal enjeksiyon sonrası 1., 3., 5., 10., 30., 35.dakikalarda, uterus insizyonu sonrası ve operasyon sonu yapılan ölçümlerde bazal değere göre istatistiksel anlamlı düşme tespit edildi. Serebral hipoksik eşik değerin altına düşen hasta sayıları açısından iki grup arasında fark saptanmadı. Sonuç : Preeklamptik gebelerde serebral hiperperfüzyonu destekler şekilde, kısmen daha yüksek bazal rSO2 değerleri gözlenebilir. Spinal anestezi sonrası, normal gebelere benzer şekilde hatta daha az oranda, hipotansiyonla korele düşme olabilir. Dolayısıyla, spinal anestezi sonrası hemodinamik değişikliğin en fazla olduğu ilk 5 dakika serebral perfüzyon açısından dikkatli olunmalıdır. Serebral mikrodolaşım bozukluğu olabilen bu gebelerde, bu düşmenin klinik etkileri ve NIRS kullanımının etkinliği ile ilgili daha fazla sayıda hasta ile yapılmış ileri çalışmalara ihtiyaç vardır. Purpose: The aim of this prospective, observational study was to compare the regional cerebral oxygen saturation (rSO2) levels measured in the preeclamptic and non-preeclamptic pregnant women undergoing cesarean section with spinal anesthesia and also to determine whether the levels of rSO2 were affected by the hemodynamic changes associated with the spinal anesthesia in the preeclamptic pregnant women. Materials and Methods: Forty-nine pregnant women, aged 18-45 years (gestational age of 32 and above), with ASA class II-III, and that were planned to undergo cesarean section with spinal anesthesia were included into the study. There were two groups including preeclampsia (Group P, n=24) and non-preeclamptic (Group N, n=25). The levels of rSO2 were monitored by the INVOS™ with two sensors in addition to routine monitoring. The levels of right and left frontal rSO2 were recorded during the surgery after the spinal anesthesia. Moreover, hemodynamic parameters, peripheral oxygen saturation, the amount of ephedrine, the total amount of blood loss, complications, 1- and 5-min Apgar scores, umbilical cord blood gas values, time from skin incision to delivery and from uterine incision to delivery for fetal assessment were recorded. Results: There was no statistically significant difference between the two groups according the basal rSO2 levels, although the levels were higher in the Group P compared to the Group N. There was a statistically significant difference between the two groups in terms of the levels of left rSO2 measured at 3 and 5 minutes after the spinal injection (the levels of left rSO2 were higher in the Group P) while there was no in terms of the levels of right rSO2. In intra-group comparisons, the levels of right rSO2 measured at 1, 3, 5 and 10 minutes after the spinal injection and after the uterine incision and delivery of the placenta were statistically significantly lower compared to the basal levels in the Group N. The levels of left rSO2 measured at 1, 3, 5 and 10 minutes after the spinal injection and after the uterine incision were statistically significantly lower compared to the basal levels. In the Group P, the levels of right and left rSO2 measured at 1, 3, 5, 10, 30 and 35 minutes after the spinal injection and after the uterine incision and at the end of surgery were statistically significantly lower compared to the basal levels. There was no significant difference between the two groups in terms of the number of patients that observed rSO2 levels below the threshold of cerebral hypoxia. Conclusion: Slightly higher basal rSO2 levels can be seen as to support cerebral hyperperfusion in preeclamptic pregnant women. After the spinal anesthesia, rSO2 levels can be decreased in correlation with hypotension in a way similar or even to a lesser extent to that of normal pregnant women. Hence, it should be careful about cerebral perfusion within the first 5 minutes after spinal anesthesia that the hemodynamic changes occur mostly. Further studies with a greater number of patients are needed to determine the clinical effects of this decrease of rSO2 levels after spinal anesthesia and to determine the utility of NIRS in the preeclamptics that may have disorders in cerebral microcirculation.
