Abstract:
Öz:AMAÇ: Silikon tüp entübasyonlu ve entübasyonsuz eksternal yaklaşımlı dakriyosistorinostomi (DSR) sonuçlarımızı değerlendirmek ve silikon tüp entübasyonunun primer ve sekonder cerrah ilerdeki yerini vurgulamak. GEREÇ VE YÖNTEM: Dupuy-Dutemps-Bourget tekniği ile geleneksel eksternal DSR uygulanan 25 göze (20 kadın, 5 erkek) silikon tüp entübe edilirken (1.grup) 51 göze (43 kadın, 8 erkek) silikon tüp entübe edilmedi (2.grup). Ortalama yaş 1.grupta 38.8 (4 ile 76 arası), 2.grupta 39.9 (20 ile 64 arası) idi. Postoperatif takip süresi 6 ile 60 ay (ortalama: 38.7) arasındadır. BULGULAR: Silikon tüplü ve tüpsüz eksternal DSR başarı oranlan sırasıyla %88 ve %88.2 idi. Başarısız kalman vakalarda silikon tüplü endoskopik revizyon cerrahisi sonrası başarı oranları 1.grupta %92, 2.grupta %96 olmak üzere ortalama %94.7'ye ulaşmıştır. Silikon tüp entübasyonunun tespit edilen başlıca komplikasyonları burun irritasyonuna bağlı gelişen huzursuzluk, erken tüp kaybı, lakrimal punktum laserasyonu ve silikon tüp etrafında granülasyon dokusu oluşmasıdır. SONUÇ: Silikon tüp entübasyonunun yapılmadığı eksternal DSR hasta için daha az rahatsız edici olmakla birlikte, silikon tüp entübasyonu özellikle revizyon vakaları ile mukozal flebin başarılı bir şekilde oluşturulmadığı veya inkomplet kalındığı vakalarda önemlidir.
Başlık (İngilizce):Comparative results of external dacryocystorhinostomy with or without silicone tube intubation
Öz (İngilizce):PURPOSE: To evaluate the results of our external approach dacryocystorhinostomy (DCR) with or without silicone tube intubation and to establish the importance of silicone tube intubation in primary and secondary surgery. MATERIAL AND METHOD: Traditional external DCR by Dupuy-Dutemps-Bourget technique with silicone tube intubation (group 1) was performed on 25 eyes (20 women and 5 men) and without silicone tube intubation (group 2) on 51 eyes (43 women and 8 men). The mean age was 38.8 years (range, 4 to 76 years-old) in group 1 and 39.9 years (range, 20 to 64 years-old) in group 2. Follow-up period is 6 to 60 months (mean, 38.7) postoperatively. RESULTS: Success rate of external DCR with or without silicone tube was found to be 88% and 88.2%, respectively. After endoscopic revision surgery with silicone tube intubation of failed cases, the success rate has reached 92% in group 1 and 96% in group 2 (mean, %94.7). The main established complications of silicone tube intubation were nasal irritation, premature tube tost, lacrimal punctum Eaceration and granulation tissue around the silicone tube. CONCLUSIONS: Although external DCR without silicone tube implantation is less annoying for the patient, the use of silicone tube is important especially for revision cases, incomplete cases and the cases that mucosal flap could not be performed successfully.
